Forms and Applications

County 4-H Record Book:  These are due each year in early June.

County Officer Registration Form:  This form is required to be filled out by all members planning to run for a county office.  The form is due no less than seven days prior to the county officer election which is held annually on the first Thursday of May. 

State Roundup Scoresheet:  This form is required for all members planning to attend State 4-H Roundup on the OSU Campus.  Points for this scoresheet are for the current year only. 

County 4-H Scoresheet:  This form is required for any member planning to apply for County Hall of Fame or the Alice Marshall Memorial Award.  It is also required for out of state 4-H trips such as Citizenship Washington Focus and the Denver Western Roundup Trip.  Due dates are on the form.
